Your success is the measure of our success, so we strip away the hassles and replace them with resources that help you deliver a higher standard of care.
You have a voice
We support our members by protecting the most important relationship of all – the one you have with your patients. Our physician-led governing committees ensure that decisions are guided by people directly involved in delivering care. You’re also surrounded by a team of associates who have spent their careers helping providers navigate the business of running a practice. They are your constant advocate, and will help you understand exactly what you need to do to be successful in the network.

Care management
We’re leading the charge to create evidence-based guidelines and meaningful clinical interventions for clinically integrated network patients. It begins by piloting programs designed to reduce readmissions and inappropriate emergency department use, treat chronic conditions like chronic obstructive pulmonary disease and diabetes, and help patients effectively manage their medications. And when we achieve success, we scale best practices across the network.
Your guide through transformation
Patient and payer expectations are redefining value in healthcare. Our Practice Transformation team develops tailored plans to help you and your staff maximize your potential for traditional and value-based compensation, without sacrificing the quality of patient care.
Connected with purpose
Every practice in the clinically integrated network must have an interoperable method of sharing clinical information with the network. Interoperability syncs the different information technology systems and software applications used by providers across the network. This is how we exchange data, analyze entire patient populations, and pinpoint opportunities for meaningful programs and interventions. Our Physician Relations team will help you choose the best solution for interoperability. Many practices use OhioHealth Link, a web-based portal that allows access to OhioHealth CareConnect for continuity of care.
Events and education
We host regular networking and education events, which are free for members and your office staff. You can also find downloadable education materials and links to helpful information in the Resource Library in your Clinician Portal.
